Ohio Department of Health Athletic Concussion Form

Starting April 26th, 2013, Ohio law (ORC 3313.539 and ORC 3314.03) prohibits schools from allowing a student to practice for or compete in interscholastic athletics (school sports) until the student has submitted this signed form stating that the student and the student’s parent or guardian have received the information contained in the Concussion Information sheet/Form.

OHSAA Athletic Physical Form

A completed physical form is required of all student-athletes before the sport season begins. Print this form for your physician to complete; then return to the athletic department. Physical forms must be on file with the school before participation can occur!

Drug and Alcohol Policy

The Tobacco / Alcohol / Drug Policy for Extracurricular Activities and Interscholastic Athletics explains the policies of the school district with regard to substance abuse by students representing the school in athletic and extracurricular contests.

Permission for Treatment

Should a medical emergency occur during an athletic contest, practice, or event, we will make every effort to contact the student’s parent prior to treatment. In the event that the parent cannot be reached, the Permission for Treatment form must be on file for the student to receive the needed medical attention.
